Abstract
Purpose: To report intravascular papillary endothelial hyperplasia of the palpebral conjunctiva originating from eyelid varix.

Case summary: A 26-year-old male without history of trauma or specific medical history was presented with a left upper eyelid mass, which was noticed 1 year prior and the mass increased which was more prominent when patient face was turned downward or coughing and stooping. Histopathological examination revealed findings of intravascular papillary endothelial hyperplasia. There has been no local recurrence during the 18 months follow-up.

Conclusions: According to previous reports intravascular papillary endothelial hyperplasia showed no change in size. In this case the mass size varied depending on the posture, this is a representative clinical manifestation of eyelid varix which was believed as one of origins of intravascular papillary endothelial hyperplasia.
